SMS Terms & Consent
These terms apply when you provide your mobile number and consent to receive text messages from ("Forever Health Rx").
Types of messages
- Transactional and care-related messages (visit status, provider messages waiting in the portal, shipping and delivery updates, appointment and billing reminders). These are sent when you provide your mobile number and consent to receive them during sign-up.
- Promotional messages (offers and news). These are sent only if you separately opt in and can be stopped at any time without affecting transactional messages.
Frequency and cost
Message frequency varies with your care and account activity. Message and data rates may apply according to your mobile plan. Carriers are not liable for delayed or undelivered messages.
Opting out and help
Reply STOP to any message to stop receiving that type of message; you will receive a one-time confirmation. Reply HELP for assistance or contact . Opting out of promotional messages does not stop transactional messages needed to deliver your care unless you also opt out of those.
Privacy
Text messages are not fully secure and may be seen by anyone with access to your phone. We keep the content of text messages minimal and direct you to the patient portal for clinical details. Your mobile number is handled as described in our Privacy Policy and is not shared with third parties for their marketing.
