Understanding PT-141 Injection

What to know first.

Regulatory status

Not FDA-approved for any use. The only FDA-approved bremelanotide product, VYLEESI®, is approved only for premenopausal women with acquired, generalized hypoactive sexual desire disorder (HSDD); its label says it is not indicated for HSDD in postmenopausal women or in men and is not indicated to enhance sexual performance. Compounded bremelanotide is not that product, and the FDA has not reviewed it. A licensed provider reviews your health history, heart health, blood pressure and current medicines, explains what is and is not known, and decides whether it may be appropriate for you.

How it works

Bremelanotide is a melanocortin receptor agonist that activates several melanocortin receptors, including receptors in the brain and in skin pigment cells, which is why skin darkening can occur. In the FDA label for the approved bremelanotide product, how it works on low sexual desire is described as unknown, and whether it helps, and how much, varies from person to person. Compounded bremelanotide injection is given under the skin; your provider sets the dose and how often it may be used, and the pharmacy label gives your directions.

Who it may be for

A provider may consider compounded bremelanotide for some adults with distressing low sexual desire, after other possible causes such as medical conditions, medicines, mood or relationship concerns have been considered, and after assessing heart health and confirming that blood pressure is well controlled. In the FDA label for the approved bremelanotide product, it must not be used with uncontrolled high blood pressure or known cardiovascular disease, it is not recommended for people at high cardiovascular risk, and its safety and effectiveness have not been established in children or older adults. Your provider decides.

Off-label use. Bremelanotide is not FDA-approved for use in men or in postmenopausal women, so prescribing it for them is outside any FDA-approved use and is decided by your provider.

Good questions. Clear answers.

How is compounded bremelanotide different from VYLEESI®?

VYLEESI® is the FDA-approved bremelanotide product, an autoinjector approved only for premenopausal women with acquired, generalized HSDD. Compounded bremelanotide is prepared by a state-licensed pharmacy for one patient under a prescription. It is not FDA-approved, the FDA does not review it for safety, effectiveness or quality, and it is not a generic version of, or equivalent to, VYLEESI® or any other branded medication. Your provider can explain what this means before any decision is made.

Can men or women after menopause use bremelanotide?

No bremelanotide product is FDA-approved for men or for women after menopause; the label for the approved product says it is not indicated for them. Bremelanotide was given to men in earlier clinical studies, but its safety and effectiveness in men have not been established through FDA review. If a provider considers it for you, that use is off-label, and they will explain what is and is not known before any decision.

Why does blood pressure matter?

In the FDA label for the approved bremelanotide product, each dose temporarily raises blood pressure and lowers heart rate, and these changes usually return to baseline within about twelve hours. Using it more often than prescribed can raise blood pressure and heart risks. It must not be used with uncontrolled high blood pressure or known cardiovascular disease and is not recommended for people at high cardiovascular risk, so your provider reviews your heart health and blood pressure before and during treatment.

Can bremelanotide darken my skin?

It can. In the FDA label for the approved bremelanotide product, focal hyperpigmentation, including on the face, gums and breasts, was reported in a small share of patients in the main trials and was much more frequent with daily use in a separate study. It is more likely in people with darker skin, and it did not fully fade in everyone after stopping. Tell your provider about any skin changes; the label says to consider stopping if darkening develops.

What side effects have been reported?

In the FDA label for the approved bremelanotide product, nausea was the most common side effect and can be severe; it happened most often after the first dose and usually lasted about two hours. Taking ondansetron beforehand did not prevent it and is not recommended. Flushing, injection site reactions, headache and vomiting were also common. These are not all the possible side effects. Report side effects to the FDA at 1-800-FDA-1088 or www.fda.gov/medwatch. In an emergency, call 911.

Does bremelanotide interact with other medicines?

In the FDA label for the approved bremelanotide product, it can slow stomach emptying, which may delay or reduce the absorption of medicines taken by mouth. The label says to avoid it with oral naltrexone taken for alcohol or opioid dependence, because lower naltrexone levels could lead to treatment failure, and with oral medicines that need a threshold level to work, such as some antibiotics. Share a complete list of everything you take with your provider.

Does completing an online visit mean I will be prescribed bremelanotide?

No. You complete a health assessment online and a licensed provider reviews your answers, history, blood pressure and current medicines, then decides whether compounded bremelanotide, another option or no treatment is right for you. Forever Health Rx is a telemedicine platform, not a pharmacy, and completing an assessment or paying does not mean you will receive a prescription. Not everyone qualifies, and individual results vary.

Important Safety Information — Bremelanotide (PT-141) Injection

Compounded drug notice

Compounded bremelanotide is prepared for an individual patient by a state-licensed pharmacy under a prescription from a licensed provider. Compounded drugs are not FDA-approved, and the FDA does not review them for safety, effectiveness or quality. Compounded bremelanotide is not a generic version of, or equivalent to, VYLEESI® or any other branded medication. Prescription required. Not everyone qualifies. Individual results vary.

Off-label use. Bremelanotide is not FDA-approved for use in men or in postmenopausal women, so prescribing it for them is outside any FDA-approved use and is decided by your provider.

Do not use if

  • You have high blood pressure that is not controlled (a contraindication in the FDA label for the approved bremelanotide product)
  • You have known cardiovascular (heart or blood vessel) disease (also a contraindication in that label)
  • You are allergic to bremelanotide or any ingredient in the compounded preparation

Before starting, tell your provider if

  • High blood pressure, heart problems or other cardiovascular risk factors
  • Kidney or liver problems
  • Darker skin, or any prior changes in skin pigmentation, because focal hyperpigmentation is more likely
  • Oral naltrexone taken for alcohol or opioid dependence, and any oral medicine that needs a quick onset or a threshold level to work, such as antibiotics or pain relievers like indomethacin
  • Every prescription and over-the-counter medicine, vitamin and herbal supplement
  • Pregnancy, plans to become pregnant, the birth control you use, or breastfeeding

Serious side effects — get medical help right away

  • Temporary rise in blood pressure and drop in heart rate. In the FDA label for the approved bremelanotide product, this occurs after each dose and usually resolves within about twelve hours, and using it more often than prescribed can raise blood pressure and heart risks. It is not recommended for people at high cardiovascular risk. If you think you are having a medical emergency, call 911.
  • Focal hyperpigmentation (skin darkening). Darkening of the face, gums or breasts can occur, more often with darker skin and with frequent use, and may not fully resolve after stopping. Tell your provider about any skin changes.
  • Nausea. The most common side effect in the FDA label for the approved bremelanotide product, sometimes severe; most often after the first dose and usually lasting about two hours. Pre-treatment with ondansetron was not found effective and is not recommended. Tell your provider about nausea that is severe or does not go away.
  • Slowed absorption of oral medicines, including naltrexone. Bremelanotide may slow stomach emptying and lower the absorption of medicines taken by mouth. The label says to avoid it with oral naltrexone for alcohol or opioid dependence, because treatment failure can have severe consequences.

Common side effects

Common side effects listed in the FDA label for the approved bremelanotide product: nausea, flushing, injection site reactions, headache, vomiting, hot flush, nasal congestion, cough, tingling, fatigue and dizziness. These are not all the possible side effects.

You may report side effects to the FDA at 1-800-FDA-1088 or www.fda.gov/medwatch.

Pregnancy and breastfeeding

In the FDA label for the approved bremelanotide product, animal studies suggest a potential for harm to an unborn baby, and the few pregnancies in clinical trials are not enough to know the risk in people. The label advises effective contraception during use and stopping if pregnancy is suspected. It is not known whether bremelanotide passes into breast milk. Tell your provider if you are pregnant, could become pregnant or are breastfeeding.
