Escitalopram
Care for depression and generalized anxiety
Escitalopram is an FDA-approved selective serotonin reuptake inhibitor (SSRI) for major depressive disorder and generalized anxiety disorder. It carries a boxed warning on suicidal thoughts and behaviors in young people, and it is prescribed online only if a licensed provider decides it is right for you. Escitalopram is an FDA-approved SSRI; if a licensed provider decides it is right for you, here is what it is approved to treat and how care works:
Approved for depression
FDA-approved to treat major depressive disorder in adults.
Generalized anxiety disorder
Also FDA-approved to treat generalized anxiety disorder in adults.
How it is thought to work
It is thought to block serotonin reuptake by nerve cells in the brain, which increases serotonin activity.
Screened and followed up
A licensed provider screens, sets the dose and follows up through the portal; not for crisis care.
Not for everyone
It carries a boxed warning on suicidal thoughts in young people; your provider decides whether it fits you.
Not medical advice. Individual results vary. A licensed provider decides whether a treatment is right for you.
Understanding Escitalopram
What to know first.
Approved use
Escitalopram is FDA-approved to treat major depressive disorder (MDD) in adults and adolescents 12 years and older, and generalized anxiety disorder (GAD) in adults and children 7 years and older. It is not approved for children younger than 7.
How it works
Escitalopram is thought to work by blocking the reuptake of serotonin, a chemical messenger, by nerve cells in the brain, which increases serotonin activity. How much it helps varies from person to person, and it can take time to notice a difference. Your provider sets the dose, and the label calls for close monitoring, especially during the first few months and whenever the dose changes. Because any psychoactive medicine may affect judgment, thinking or motor skills, use caution with driving or machinery until you know how it affects you. If stopping is ever needed, the dose is usually lowered gradually.
Who it may be for
A provider may consider escitalopram for adults with major depressive disorder or generalized anxiety disorder, after screening for bipolar disorder and reviewing every medicine you take. It is not for people taking, or who recently stopped, an MAOI antidepressant, people taking pimozide, or anyone allergic to escitalopram or citalopram, and it is used with caution after a recent heart attack, with unstable heart disease or with liver impairment. Forever Health Rx does not provide crisis or emergency care. Your provider decides.
How it works
Three steps, all online.
01
Complete your online visit
Answer questions about your health history, medications and goals. Submitting a visit does not guarantee a prescription.
02
A licensed provider reviews
A provider licensed in your state decides whether treatment is appropriate for you, or recommends a different path.
03
If prescribed, delivered
A licensed pharmacy dispenses your medication and ships it to your door. Follow-up continues through the patient portal.
Escitalopram is FDA-approved for major depressive disorder in adults and adolescents 12 and older, and for generalized anxiety disorder in adults and children 7 and older. It is not approved for children under 7. A licensed provider reviews your symptoms, history and other medicines to decide whether escitalopram may be appropriate for you, and the conditions evaluated online may be narrower than the full label.
What should I know about the boxed warning?
Antidepressants increased the risk of suicidal thoughts and behaviors in children, adolescents and young adults in short-term studies. Anyone starting an antidepressant should be watched closely for worsening mood or new suicidal thoughts, especially in the first few months and when the dose changes, and family or caregivers can help notice changes. If you are in crisis or thinking about suicide, call or text 988 or call 911.
Can I stop taking escitalopram on my own?
Please do not stop suddenly. Reports after abrupt discontinuation of SSRIs include low mood, irritability, agitation, dizziness, electric shock-like sensations, anxiety, confusion, headache, tiredness, emotional ups and downs, trouble sleeping and hypomania. These symptoms are usually self-limiting, but serious cases have been reported. The label recommends a gradual reduction whenever possible, and your provider can plan the safest way to stop if that is the right choice.
Which medicines should I tell my provider about?
All of them, including supplements. Escitalopram must not be combined with MAOIs (including linezolid or methylene blue given into a vein) or pimozide. Triptans such as sumatriptan, tricyclic antidepressants, opioids such as fentanyl, tramadol, meperidine and methadone, lithium, tryptophan, buspirone, amphetamines and St. John's wort raise the risk of serotonin syndrome. Aspirin, NSAIDs, warfarin and other blood thinners raise the risk of bleeding.
Can escitalopram affect sexual function?
It can. The label notes that SSRIs may cause sexual side effects: in men, delayed or failed ejaculation, decreased desire and erectile problems; in women, decreased desire and delayed or absent orgasm. These changes may not come up unless someone asks, so it helps to tell your provider about anything you notice. Your provider can discuss options with you.
Is this service for emergencies?
No. Forever Health Rx is a telemedicine platform for non-urgent care and does not provide crisis or emergency care. If you are in crisis or thinking about suicide, call or text 988 (Suicide & Crisis Lifeline), which is free, confidential and available 24/7, or call 911. If someone is in immediate danger, call 911 right away.
How does the online visit work?
You complete a health assessment online and a licensed provider reviews your answers, history and current medicines. If they decide escitalopram is appropriate, a licensed pharmacy dispenses it. Forever Health Rx is a telemedicine platform, not a pharmacy, and completing an assessment or paying does not mean you will receive a prescription. Not everyone qualifies, and individual results vary.
Important Safety Information — Escitalopram
Warning: Suicidal thoughts and behaviors
Antidepressants increased the risk of suicidal thoughts and behaviors in pediatric and young adult patients in short-term studies. Everyone who takes an antidepressant should be monitored closely for clinical worsening and for the emergence of suicidal thoughts and behaviors, especially during the first few months and whenever the dose changes. Families and caregivers should watch for changes and tell the prescriber. Escitalopram is not approved for children younger than 7.
What is it approved for?
Escitalopram is FDA-approved to treat major depressive disorder (MDD) in adults and adolescents 12 years and older, and generalized anxiety disorder (GAD) in adults and children 7 years and older. It is not approved for children younger than 7.
Do not use if
Taking an MAOI antidepressant, or having stopped one within the past 14 days (including linezolid or methylene blue given into a vein)
Taking pimozide
Known allergy to escitalopram, citalopram or any ingredient in the tablets
Before starting, tell your provider if
Personal or family history of bipolar disorder, mania or hypomania
Seizures, glaucoma or narrow eye angles, a recent heart attack or unstable heart disease, low sodium, or liver problems
Bleeding problems, or use of aspirin, NSAIDs, other antiplatelet medicines, warfarin or other anticoagulants
Other medicines that affect serotonin: triptans (including sumatriptan), tricyclic antidepressants, fentanyl, meperidine, methadone, lithium, tramadol, tryptophan, buspirone, amphetamines or St. John's wort
Being older, taking diuretics, or being dehydrated, which raise the risk of low sodium
Pregnancy, plans to become pregnant, or breastfeeding
Serious side effects — get medical help right away
Serotonin syndrome. A potentially life-threatening reaction, more likely with other serotonin-affecting medicines but possible with escitalopram alone. Get help right away for agitation, hallucinations, confusion, a fast heartbeat, high or low blood pressure, sweating or fever, flushing, tremor, muscle stiffness or twitching, loss of coordination, seizures, nausea, vomiting or diarrhea.
Increased risk of bleeding. Bruising, nosebleeds and more serious bleeding can occur, especially with aspirin, NSAIDs, other antiplatelet medicines, warfarin or other medicines that affect clotting.
Manic episodes. In people with bipolar disorder an antidepressant may trigger a manic or mixed episode. Report greatly increased energy, racing thoughts, unusually grand ideas, severe trouble sleeping, reckless behavior, excessive happiness or irritability, or talking more or faster than usual.
Angle-closure glaucoma. In people with untreated narrow eye angles, antidepressants can trigger an angle-closure attack. Get care right away for eye pain, changes in vision, or swelling or redness in or around the eye.
Low sodium (hyponatremia). Often linked to a hormone imbalance (SIADH) and usually reversible when the medicine is stopped. Signs include headache, difficulty concentrating, memory problems, confusion, weakness and unsteadiness that can lead to falls; severe cases can cause hallucinations, fainting, seizures or coma. Older adults and people taking diuretics are at greater risk.
Seizures. Escitalopram is prescribed with caution in people with a history of seizures.
Discontinuation symptoms. Do not stop suddenly. Stopping abruptly can cause low mood, irritability, agitation, dizziness, electric shock-like sensations, anxiety, confusion, headache, tiredness, emotional ups and downs, trouble sleeping and hypomania.
Common side effects
Common side effects: trouble sleeping, ejaculation problems (mainly delayed ejaculation), nausea, increased sweating, tiredness and sleepiness, decreased sexual desire and difficulty reaching orgasm. These are not all the possible side effects.
You may report side effects to the FDA at 1-800-FDA-1088 or www.fda.gov/medwatch.
Pregnancy and breastfeeding
Talk with your provider before starting, stopping or changing escitalopram during pregnancy, because untreated depression also has risks. Published studies have not established an increased risk of major birth defects or miscarriage. SSRI exposure during pregnancy carries risks of persistent pulmonary hypertension of the newborn (PPHN) and poor adaptation in the newborn, and use in the month before delivery has been associated with a modest increase in the risk of heavy bleeding after birth. Escitalopram passes into breast milk, and excessive sedation, restlessness, agitation, poor feeding and poor weight gain have been reported in breastfed infants, so tell your provider if you are breastfeeding.
If you are in crisis or thinking about suicide, call or text 988 (Suicide & Crisis Lifeline) or call 911.
Forever Health Rx does not provide crisis or emergency care.
Call your provider right away about new or worse depression, anxiety or panic attacks, agitation, restlessness, anger or irritability, acting aggressive or on dangerous impulses, trouble sleeping, or other unusual changes in mood or behavior.
Prescription only. Escitalopram is available only after an online visit with a licensed provider, who determines whether treatment is appropriate for you. This summary does not replace the full prescribing information. Not medical advice. Results may vary.
Forever Health Rx does not provide emergency medical care. Call 911 or go to the nearest emergency department if you are experiencing a medical emergency. For urgent questions about a serious side effect, message your care team through the patient portal and seek in-person care if symptoms are severe.
