What it may help with

The benefits of Tadalafil

Tadalafil is a prescription treatment for erectile dysfunction (ED). With sexual stimulation, it can help you:

  • Firmer erections

    Get and keep an erection firm enough for sex.

  • Up to 36 hours

    An as-needed dose can work for up to 36 hours.

  • Daily option

    A daily dose means no planning around timing.

  • Works with arousal

    It works when you're sexually aroused, so you stay in control of the moment.

Individual results vary. Benefits are not guaranteed and depend on your provider's evaluation and your plan.

Understanding Tadalafil

What to know first.

Approved use

Tadalafil tablets are FDA-approved to treat erectile dysfunction (ED), the signs and symptoms of benign prostatic hyperplasia (BPH, an enlarged prostate), and both conditions together. The label does not recommend tadalafil together with alpha-blockers for BPH. Tadalafil is not indicated for use in women. A different tadalafil product is FDA-approved for pulmonary arterial hypertension (high blood pressure in the lungs); the two must not be combined.

How it works

Tadalafil belongs to a group of medicines called PDE5 inhibitors. During sexual arousal the body releases nitric oxide, which raises a natural messenger (cGMP) that relaxes muscle tissue in the penis so blood can flow in; tadalafil slows the breakdown of that messenger, and it has no effect without sexual stimulation. Tadalafil stays in the body longer than some other PDE5 inhibitors: its label reports a half-life of about 17.5 hours in healthy adults and notes that erectile function may be improved for up to 36 hours after a dose. Response varies from person to person.

Who it may be for

A licensed provider may consider tadalafil for adult men with erectile dysfunction, or with urinary symptoms of an enlarged prostate, after reviewing their heart health, blood pressure, kidney and liver function and current medicines. It is not for anyone who takes nitrates or riociguat, for whom sexual activity is not advisable because of heart health, or who has severe liver disease. Your provider decides.

FAQ

Good questions. Clear answers.

Is tadalafil FDA-approved, and how does the online visit work?

Yes. Tadalafil tablets are FDA-approved for erectile dysfunction, for the signs and symptoms of an enlarged prostate (BPH), and for both together. You complete a secure online health assessment; a licensed provider reviews it, may ask follow-up questions, and decides whether tadalafil is appropriate. Completing the assessment does not mean you will receive a prescription. If it is prescribed, a licensed pharmacy dispenses an FDA-approved tadalafil product and ships it to you, and your provider sets the dose.

How is tadalafil different from sildenafil?

Both are FDA-approved PDE5 inhibitors for erectile dysfunction, and both need sexual stimulation to work. Tadalafil is also FDA-approved for the signs and symptoms of BPH, and it stays in the body longer: its label reports a half-life of about 17.5 hours and effects measured for up to 36 hours after a dose. That longer presence also means interactions last longer. Your provider weighs your health history, other medicines and preferences and decides which option, if any, may suit you.

Can I take tadalafil with nitrates or blood-pressure medicines?

Not with nitrates. Taking tadalafil with any nitrate medicine, such as nitroglycerin, can lower blood pressure dangerously, and the same applies to riociguat. Because tadalafil stays in the body for a long time, if you ever need emergency care for a heart problem it is important to tell the care team when you last took it. Alpha-blockers and other blood-pressure medicines can add to the blood-pressure-lowering effect; the label does not recommend combining tadalafil with alpha-blockers to treat BPH.

Can I drink alcohol while taking tadalafil?

Be careful. Alcohol and tadalafil both widen blood vessels a little, so drinking a lot of alcohol while taking tadalafil can increase the chance of a drop in blood pressure when you stand up, with symptoms such as dizziness or light-headedness. The label advises against drinking too much alcohol. Talk with your provider about what is sensible for you, especially if you also take blood-pressure medicines.

What should I do if an erection lasts more than four hours?

Get emergency medical help right away. An erection that lasts more than four hours, painful or not, is called priapism and needs urgent treatment; without prompt care it can permanently damage the penis. Before starting, tell your provider if you have a deformed penis shape or Peyronie's disease, or a condition that can make priapism more likely, such as sickle cell anemia, multiple myeloma or leukemia.

What about vision or hearing changes?

Rarely, men taking PDE5 inhibitors have reported sudden loss of vision in one or both eyes, which may be a sign of an eye problem called NAION and can be permanent, or a sudden decrease or loss of hearing. If either happens, stop taking tadalafil and get medical help right away. Tell your provider beforehand if you have ever had NAION or other serious eye problems. Blockage of a retinal vein or artery and visual field defects have also been reported after marketing.

What are the common side effects?

The most common side effects are headache, indigestion, back pain, muscle aches, stuffy nose, flushing and pain in the arms or legs. In studies, back pain and muscle aches generally began 12 to 24 hours after a dose and typically eased within 48 hours. Tell your provider about any side effect that bothers you or does not go away, and call 911 in an emergency.

Important Safety Information — Tadalafil

What is it approved for?

Tadalafil tablets are FDA-approved to treat erectile dysfunction (ED), the signs and symptoms of benign prostatic hyperplasia (BPH, an enlarged prostate), and both conditions together. The label does not recommend tadalafil together with alpha-blockers for BPH. Tadalafil is not indicated for use in women. A different tadalafil product is FDA-approved for pulmonary arterial hypertension (high blood pressure in the lungs); the two must not be combined.

Do not use if

  • take any nitrate medicine, such as nitroglycerin, in any form;
  • take a guanylate cyclase stimulator such as riociguat;
  • have had a serious allergic reaction to tadalafil, including to tadalafil products for pulmonary arterial hypertension (severe skin reactions such as Stevens-Johnson syndrome and exfoliative dermatitis have been reported);
  • have been advised that sexual activity is not safe for you because of your heart health.

Before starting, tell your provider if

  • have or have had heart problems such as a heart attack, angina, heart failure or an irregular heartbeat, or have had a stroke;
  • have low blood pressure, or high blood pressure that is not controlled;
  • have ever had severe vision loss, including an eye problem called NAION, or have other serious eye problems;
  • have liver problems (tadalafil is not recommended in severe liver disease) or kidney problems, or are on dialysis;
  • have a bleeding problem or active stomach ulcers;
  • have a deformed penis shape or Peyronie's disease, have had an erection that lasted more than four hours, or have sickle cell anemia, multiple myeloma or leukemia;
  • have urinary symptoms: other conditions can cause similar symptoms, and prostate cancer and BPH may exist together, so your provider may check for them first;
  • take an alpha-blocker or other blood-pressure medicine, another PDE5 inhibitor or ED treatment (including tadalafil products for pulmonary arterial hypertension), or medicines such as ritonavir or ketoconazole;
  • drink alcohol regularly or heavily.

Serious side effects — get medical help right away

  • An erection that will not go away (priapism). If an erection lasts more than four hours, painful or not, get emergency medical help right away to help prevent lasting damage to the penis.
  • Sudden vision loss in one or both eyes. Stop taking tadalafil and get medical help right away. It may be a sign of an eye problem called NAION, which has been reported rarely with PDE5 inhibitors and can cause permanent vision loss.
  • Sudden decrease or loss of hearing. Stop taking tadalafil and get medical help promptly.
  • Heart problems. Heart attack, sudden cardiac death, stroke, chest pain, palpitations and fast heartbeat have been reported after marketing; it is not known whether tadalafil, sexual activity, underlying heart disease or other factors caused them. If you have chest pain after taking a dose, get medical help right away.
  • Low blood pressure. Taking tadalafil with an alpha-blocker, other blood-pressure medicines or a lot of alcohol can lower blood pressure further, which may cause dizziness or fainting, especially when standing up.
  • Serious skin and allergic reactions. Hives and severe skin reactions, including Stevens-Johnson syndrome and exfoliative dermatitis, have been reported. Stop taking tadalafil and get medical help right away for a spreading rash, blistering or peeling skin, or swelling of the face, lips or throat.
  • Other reported effects. Migraine, seizures and transient global amnesia (a sudden, temporary loss of memory) have been reported after marketing. Tell your provider about any unusual symptom.
