What it may help with

The benefits of Enclomiphene

Compounded enclomiphene is an oral capsule that is not FDA-approved; if a licensed provider prescribes it, here is what to know:

  • Not FDA-approved, compounded

    No enclomiphene product is FDA-approved; this is a compounded medicine, and your provider decides if it fits you.

  • Related to clomiphene

    Enclomiphene is one of the forms (isomers) that make up clomiphene, an FDA-approved medicine.

  • How it is thought to work

    A selective estrogen receptor modulator thought to block estrogen's feedback signal in the brain's hormone centers.

  • Provider reviews first

    Your provider reviews your health history, current medicines and, where appropriate, lab tests before deciding.

  • Follow-up in your portal

    Questions and follow-up go through the patient portal; if prescribed, a licensed pharmacy ships it to you.

Not medical advice. Individual results vary. A licensed provider decides whether a treatment is right for you.

Understanding Enclomiphene

What to know first.

Regulatory status

Not FDA-approved for any use. No enclomiphene product has been approved by the FDA; an application for one was submitted to the FDA and was not approved. A licensed provider reviews your health history, current medicines and, where appropriate, lab tests before deciding whether it may be appropriate for you.

How it works

Enclomiphene is one of the two closely related forms (isomers) that make up clomiphene, an FDA-approved medicine. It is a selective estrogen receptor modulator (SERM): it is thought to block estrogen's feedback signal in the hypothalamus and pituitary, the parts of the brain that help regulate the body's reproductive hormones. How a person responds varies, and its long-term safety has not been established.

Who it may be for

A licensed provider may consider it for some adult men after reviewing their health history and, where appropriate, lab tests. It has been studied in men only, and it may not be suitable for people with liver disease, a history of blood clots, heart disease, or certain pituitary, thyroid or adrenal conditions. Your provider decides.

FAQ

Good questions. Clear answers.

Is enclomiphene FDA-approved?

No. Enclomiphene is not FDA-approved for any use, and no enclomiphene product has been approved by the FDA. An application for an enclomiphene product was submitted to the FDA and was not approved. Compounded enclomiphene is prepared for an individual patient under a prescription, and the FDA does not review compounded drugs for safety, effectiveness or quality.

How is enclomiphene different from clomiphene?

Clomiphene is an FDA-approved medicine made of two closely related forms, called isomers: enclomiphene and zuclomiphene. Enclomiphene is one of those forms on its own. Clomiphene is approved to treat ovulation problems in women who want to become pregnant; it is not approved for men. Compounded enclomiphene is a different product from clomiphene and is not a substitute for it.

What does the evidence show?

The evidence is limited. FDA reviewers who looked at the published studies in 2022 concluded that, although some hormone levels changed, the studies did not show a meaningful improvement in symptoms, and safety and long-term data were limited. Results vary from person to person. Your provider will talk with you about what is and is not known before recommending anything.

What side effects have been reported?

In studies, the side effects reported most often included headache, hot flushes, nausea, muscle spasms and dizziness. FDA reviewers also listed heart and blood-clot events, and changes in some blood tests such as estradiol, PSA and hematocrit, as safety concerns. Some people stopped treatment because of blurred vision. Tell your provider about any side effect, and call 911 in an emergency.

Who should not take enclomiphene?

Enclomiphene has been studied in men only. The label for clomiphene, the related FDA-approved medicine, says it should not be used by people with liver disease or a history of liver problems, uncontrolled thyroid or adrenal problems, or a pituitary tumor or other growth in the brain. Tell your provider about any history of blood clots, stroke, heart disease, eye problems or prostate concerns. Your provider decides.

How does the online visit work?

You complete a secure online health assessment. A licensed provider reviews your answers, may ask follow-up questions or request lab tests, and decides whether enclomiphene may be appropriate. Completing the assessment does not mean you will receive a prescription. If it is prescribed, a state-licensed pharmacy prepares and ships it, and your provider sets the dose and follows up with you.

Important Safety Information — Enclomiphene

Compounded drug notice

Compounded enclomiphene is prepared for an individual patient by a state-licensed pharmacy under a prescription from a licensed provider. Compounded drugs are not FDA-approved, and the FDA does not review them for safety, effectiveness or quality. Compounded enclomiphene is not a generic version of, or equivalent to, any branded medication, and no enclomiphene product is FDA-approved. Prescription required. Not everyone qualifies. Individual results vary.

Do not use if

  • are allergic to enclomiphene, clomiphene or any ingredient in the capsule;
  • have liver disease or a history of liver problems (a contraindication on the label of clomiphene, the related FDA-approved medicine);
  • have uncontrolled thyroid or adrenal problems (also a contraindication on the clomiphene label);
  • have a pituitary tumor or other growth in the brain (also a contraindication on the clomiphene label).

Before starting, tell your provider if

  • have ever had a blood clot in a leg or lung, a stroke or a mini-stroke (transient ischemic attack);
  • have heart disease or an irregular heartbeat;
  • have liver or kidney problems (people with these conditions had higher levels of the medicine in their blood in studies);
  • have cataracts or other eye problems, or notice any change in your vision;
  • have a history of prostate problems or a raised PSA, or a high red blood cell count;
  • take any other prescription or over-the-counter medicines, vitamins or supplements (enclomiphene is broken down in the body mainly by the liver enzyme CYP2D6).

Serious side effects — get medical help right away

  • Heart and blood-clot events. FDA reviewers listed heart and blood-clot (thromboembolic) events as a safety concern, and blood clots in the legs and lungs were reported in a six-month enclomiphene safety study. Get emergency help right away for chest pain, sudden shortness of breath or signs of a stroke.
  • Vision changes. Blurred vision led some people to stop enclomiphene in studies. The clomiphene label warns that blurring, spots or flashes can occur and may be prolonged and possibly permanent. Stop and contact your provider right away for an eye check if your vision changes.
  • Changes in blood tests. FDA reviewers noted increases in estradiol, PSA and hematocrit (the share of red blood cells in the blood) as safety concerns. Your provider may order blood tests to check these.
  • Mood changes. Aggression led some people to stop treatment in studies, and anxiety and depression were also reported. Tell your provider about any change in mood or behavior.

Common side effects

Common side effects: headache, hot flushes, nausea, muscle spasms and dizziness were among those reported most often in studies. These are not all the possible side effects.

You may report side effects to the FDA at 1-800-FDA-1088 or www.fda.gov/medwatch.

Pregnancy and breastfeeding

Enclomiphene has been studied in men only and is not for use by women. The label for clomiphene, the related FDA-approved medicine, says it must not be used during pregnancy.

The long-term safety of enclomiphene has not been established. In 2018 the European Medicines Agency declined to authorize an enclomiphene product because its safety and effectiveness had not been sufficiently shown.

Prescription only. Enclomiphene is available only after an online visit with a licensed provider, who determines whether treatment is appropriate for you. This summary does not replace the full prescribing information. Not medical advice. Results may vary.

Forever Health Rx does not provide emergency medical care. Call 911 or go to the nearest emergency department if you are experiencing a medical emergency. For urgent questions about a serious side effect, message your care team through the patient portal and seek in-person care if symptoms are severe.
