Understanding PT-141 Nasal Spray

What to know first.

Regulatory status

Not FDA-approved for any use. No nasal form of bremelanotide is FDA-approved. The only FDA-approved bremelanotide product, VYLEESI®, is an injection under the skin approved only for premenopausal women with acquired, generalized hypoactive sexual desire disorder (HSDD); its label says it is not indicated for postmenopausal women or men and is not indicated to enhance sexual performance. Compounded bremelanotide nasal spray is not that product, and the FDA has not reviewed it. A licensed provider reviews your health history, heart health, blood pressure and current medicines, explains what is and is not known, and decides whether it may be appropriate for you.

How it works

Bremelanotide is a melanocortin receptor agonist that activates several melanocortin receptors, including receptors in the brain and in skin pigment cells, which is why skin darkening can occur. With a nasal spray the medicine is taken up through the nose; its developer reported wide variation in blood levels after nasal use, and a later published study described nasal absorption as less predictable than injection under the skin. Whether it helps, and how much, varies from person to person, and your provider sets the dose and how often it may be used.

Who it may be for

A provider may consider compounded bremelanotide nasal spray for some adults with distressing low sexual desire, after other possible causes such as medical conditions, medicines, mood or relationship concerns have been considered, and after assessing heart health and confirming that blood pressure is well controlled, which matters even more because blood pressure increases were reported with nasal bremelanotide. In the FDA label for the approved bremelanotide product, it must not be used with uncontrolled high blood pressure or known cardiovascular disease and is not recommended for people at high cardiovascular risk. Your provider decides.

Off-label use. No bremelanotide product is FDA-approved for nasal use, for men or for postmenopausal women, so prescribing compounded bremelanotide nasal spray is outside any FDA-approved use and is decided by your provider.

Good questions. Clear answers.

Is there an FDA-approved bremelanotide nasal spray?

No. The only FDA-approved bremelanotide product is VYLEESI®, an injection under the skin for premenopausal women with acquired, generalized HSDD. Compounded bremelanotide nasal spray is prepared by a state-licensed pharmacy for one patient under a prescription. It is not FDA-approved, the FDA does not review it for safety, effectiveness or quality, and it is not a generic version of, or equivalent to, VYLEESI® or any other branded medication.

What is known about bremelanotide used in the nose?

Its developer studied nasal bremelanotide in clinical trials in men and women. In a 2009 annual report filed with the U.S. Securities and Exchange Commission, the developer said blood pressure increases were seen in some people who received it nasally, that this was a significant factor in stopping its work on the nasal form, and that variable uptake through the nose may partly explain the blood pressure, nausea and vomiting seen. The approved product was later developed as an injection under the skin.

Can men or women after menopause use it?

No bremelanotide product is FDA-approved for men or for women after menopause, and no nasal form is approved for anyone. The label for the approved injection says it is not indicated for postmenopausal women or men. If a provider considers compounded bremelanotide nasal spray for you, that use is off-label and outside any FDA-approved use, and they will explain what is and is not known before any decision.

Why does blood pressure matter so much?

Its developer said blood pressure increases in some people were a significant factor in stopping its work on nasal bremelanotide. In the FDA label for the approved injection, each dose temporarily raises blood pressure and lowers heart rate, usually returning to baseline within about twelve hours, and it must not be used with uncontrolled high blood pressure or known cardiovascular disease. Your provider reviews your heart health and blood pressure before and during treatment.

What side effects have been reported?

Its developer reported nausea and vomiting with nasal bremelanotide. In the FDA label for the approved injection, nausea was the most common side effect and can be severe, flushing, headache and vomiting were also common, and focal skin darkening, including on the face, gums and breasts, was reported. These are not all the possible side effects. Report side effects to the FDA at 1-800-FDA-1088 or www.fda.gov/medwatch. In an emergency, call 911.

Does bremelanotide interact with other medicines?

In the FDA label for the approved bremelanotide product, it can slow stomach emptying, which may delay or reduce the absorption of medicines taken by mouth. The label says to avoid it with oral naltrexone taken for alcohol or opioid dependence, because lower naltrexone levels could lead to treatment failure, and with oral medicines that need a threshold level to work, such as some antibiotics. Share a complete list of everything you take with your provider.

Does completing an online visit mean I will be prescribed the nasal spray?

No. You complete a health assessment online and a licensed provider reviews your answers, history, blood pressure and current medicines, then decides whether compounded bremelanotide nasal spray, another option or no treatment is right for you. Forever Health Rx is a telemedicine platform, not a pharmacy, and completing an assessment or paying does not mean you will receive a prescription. Not everyone qualifies, and individual results vary.

Important Safety Information — Bremelanotide (PT-141) Nasal Spray

Compounded drug notice

Compounded bremelanotide nasal spray is prepared for an individual patient by a state-licensed pharmacy under a prescription from a licensed provider. Compounded drugs are not FDA-approved, and the FDA does not review them for safety, effectiveness or quality. Compounded bremelanotide nasal spray is not a generic version of, or equivalent to, VYLEESI® or any other branded medication, and no nasal form of bremelanotide is FDA-approved. Prescription required. Not everyone qualifies. Individual results vary.

Off-label use. No bremelanotide product is FDA-approved for nasal use, for men or for postmenopausal women, so prescribing compounded bremelanotide nasal spray is outside any FDA-approved use and is decided by your provider.

Do not use if

  • You have high blood pressure that is not controlled (a contraindication in the FDA label for the approved bremelanotide product)
  • You have known cardiovascular (heart or blood vessel) disease (also a contraindication in that label)
  • You are allergic to bremelanotide or any ingredient in the compounded preparation

Before starting, tell your provider if

  • High blood pressure, heart problems or other cardiovascular risk factors
  • Kidney or liver problems
  • Darker skin, or any prior changes in skin pigmentation, because focal hyperpigmentation is more likely
  • Oral naltrexone taken for alcohol or opioid dependence, and any oral medicine that needs a quick onset or a threshold level to work, such as antibiotics or pain relievers like indomethacin
  • Every prescription and over-the-counter medicine, vitamin and herbal supplement
  • Pregnancy, plans to become pregnant, the birth control you use, or breastfeeding

Serious side effects — get medical help right away

  • Blood pressure increases. Its developer reported blood pressure increases in some people who used nasal bremelanotide, and blood levels after nasal use varied widely from person to person. In the FDA label for the approved injection, each dose temporarily raises blood pressure and lowers heart rate, usually resolving within about twelve hours. If you think you are having a medical emergency, call 911.
  • Focal hyperpigmentation (skin darkening). In the FDA label for the approved bremelanotide product, darkening of the face, gums or breasts can occur, more often with darker skin and with frequent use, and may not fully resolve after stopping. Tell your provider about any skin changes.
  • Nausea and vomiting. Nausea and vomiting were reported with nasal bremelanotide. In the FDA label for the approved injection, nausea was the most common side effect, sometimes severe, and pre-treatment with ondansetron was not found effective and is not recommended. Tell your provider about nausea that is severe or does not go away.
  • Slowed absorption of oral medicines, including naltrexone. In the FDA label for the approved bremelanotide product, it may slow stomach emptying and lower the absorption of medicines taken by mouth. The label says to avoid it with oral naltrexone for alcohol or opioid dependence, because treatment failure can have severe consequences.

Common side effects

Common side effects: its developer reported nausea and vomiting with nasal bremelanotide. In the FDA label for the approved injection, the most common were nausea, flushing, injection site reactions, headache and vomiting. Side effects specific to nasal use have not been established through FDA review. These are not all the possible side effects.

You may report side effects to the FDA at 1-800-FDA-1088 or www.fda.gov/medwatch.

Pregnancy and breastfeeding

In the FDA label for the approved bremelanotide product, animal studies suggest a potential for harm to an unborn baby, and the few pregnancies in clinical trials are not enough to know the risk in people. The label advises effective contraception during use and stopping if pregnancy is suspected. It is not known whether bremelanotide passes into breast milk. Tell your provider if you are pregnant, could become pregnant or are breastfeeding.
