Understanding Lipo-B (MIC + B12)
What to know first.
Regulatory status
Not FDA-approved for any use. No lipotropic (MIC) injection, with or without vitamin B12, is FDA-approved for weight loss or any other use. Cyanocobalamin (vitamin B12) injection on its own is FDA-approved for vitamin B12 deficiency caused by poor absorption, such as pernicious anemia; that approval applies to the single-ingredient product, not to this compounded blend. A licensed provider reviews your health history, current medicines and goals, explains what is and is not known, and decides whether it may be appropriate for you.
How it works
Lipo-B combines compounds the body uses in normal metabolism: methionine, an essential amino acid the body uses to make S-adenosylmethionine, a compound that donates methyl groups in many chemical reactions; inositol, which takes part in cell signaling; choline, which the body needs to build cell membranes and to make acetylcholine, a chemical messenger in the nervous system, and which helps carry fats out of the liver; and vitamin B12, which the body needs for healthy red blood cells, nerve function and DNA synthesis. The name "lipotropic" refers to the role some of these nutrients play in how the liver handles fats; it is not a promise of any effect on weight. Evidence that lipotropic injections help with weight or energy is limited, the combination has not been reviewed by the FDA, and results vary. Your provider explains what is and is not known.
Who it may be for
A provider may consider Lipo-B for some adults after reviewing their health history, current medicines and goals. It is not for anyone allergic to cobalt, vitamin B12 or any ingredient in the preparation, and people with Leber's disease (a hereditary optic nerve condition) need special caution; its safety during pregnancy and breastfeeding has not been established through FDA review. Your provider decides.

Lipo-B is a compounded injection that combines methionine, inositol and choline, often called MIC, with cyanocobalamin (vitamin B12). The body uses each of these in normal metabolism. Lipo-B is prepared for an individual patient by a state-licensed pharmacy and is prescribed only if a licensed provider decides it is appropriate. The exact ingredients are listed on the pharmacy label.
Is Lipo-B FDA-approved?
No. No lipotropic (MIC) injection is FDA-approved for weight loss or any other use. Cyanocobalamin injection on its own is FDA-approved for vitamin B12 deficiency caused by poor absorption, but that approval does not extend to this compounded blend. The FDA does not review compounded drugs for safety, effectiveness or quality, and Lipo-B is not a generic version of, or equivalent to, any branded medication.
Will Lipo-B help with weight or energy?
No outcome is promised. Evidence that lipotropic injections help with weight or energy is limited, and the combination has not been reviewed by the FDA. The NIH Office of Dietary Supplements notes that vitamin B12 does not appear to improve performance or endurance in people who already have enough of it. Your provider explains what is and is not known, and results vary.
Is Lipo-B a treatment for vitamin B12 deficiency?
No. Lipo-B is not FDA-approved to treat vitamin B12 deficiency. FDA-approved cyanocobalamin injection is the product approved for B12 deficiency caused by poor absorption, and a provider confirms a deficiency with blood tests. The label for cyanocobalamin injection warns that giving B12 without a clear diagnosis can hide the true cause of anemia. Tell your provider about any anemia, B12 or folate problem.
What side effects are possible?
The side effects of the combined injection have not been well studied, and the FDA has not reviewed its safety. For vitamin B12 injection, the FDA label reports mild, short-lived diarrhea, itching, a temporary rash and, rarely, serious allergic reactions. High intakes of choline have been linked to a fishy body odor, vomiting, sweating and low blood pressure. Report side effects to the FDA at 1-800-FDA-1088 or www.fda.gov/medwatch. In an emergency, call 911.
How is Lipo-B given, and who sets the dose?
If a provider prescribes it, Lipo-B is given as an injection into a muscle. Your provider sets the dose and how often to use it, and the pharmacy label gives your directions. This page does not give dosing information. Never share needles or vials, and follow the pharmacy's storage and disposal instructions.
Does completing an online visit mean I will be prescribed Lipo-B?
No. Completing an online visit or paying does not mean a prescription will be issued. A licensed provider reviews your health history, medicines and goals and decides whether Lipo-B, another option or no treatment is right for you. If it is not appropriate, you will be told why. Not everyone qualifies.

Do not use if
You are sensitive (allergic) to cobalt or vitamin B12
You are allergic to methionine, inositol, choline or any other ingredient in the compounded preparation
Before starting, tell your provider if
You have Leber's disease (hereditary optic nerve atrophy)
You have been diagnosed with, or may have, anemia or a vitamin B12 or folate deficiency
You have heart or lung problems
You take folic acid or any other vitamins or supplements
You take antibiotics, methotrexate or pyrimethamine (which can affect B12 and folate blood tests), or colchicine or para-aminosalicylic acid (which can reduce B12 absorption)
You drink alcohol heavily (heavy drinking can reduce B12 absorption)
You have had an allergic reaction to any medicine
You are pregnant, plan to become pregnant or are breastfeeding
Serious side effects — get medical help right away
Serious allergic reactions (anaphylaxis). Anaphylactic shock and death have been reported after vitamin B12 injections. Stop using Lipo-B and get emergency help (call 911) for swelling of the face, lips, tongue or throat, trouble breathing or swallowing, or a widespread rash or hives.
Optic nerve damage in Leber's disease. People with early Leber's disease (hereditary optic nerve atrophy) who were treated with cyanocobalamin have had severe and rapid optic nerve damage.
Low blood potassium (hypokalemia). Low potassium and sudden death may occur when severe megaloblastic anemia is treated intensively with vitamin B12. Tell your provider if you have been diagnosed with anemia.
Heart and blood-vessel problems. Fluid in the lungs (pulmonary edema) and congestive heart failure early in treatment, and blood clots in the arms or legs (peripheral vascular thrombosis), have been reported with vitamin B12 injection. Tell your provider right away about any new breathing problem or swelling; in an emergency, call 911.
Increase in red blood cells. Polycythemia vera, a condition with too many red blood cells, has been reported with vitamin B12 injection.
Common side effects
Common side effects: the side effects of the combined Lipo-B injection have not been well studied, and the FDA has not reviewed its safety. For vitamin B12 injection, the FDA label reports mild, short-lived diarrhea, itching, a temporary rash and a feeling of swelling of the whole body. High intakes of choline have been linked to a fishy body odor, vomiting, excessive sweating and salivation, low blood pressure and liver problems. Tell your provider about any reaction where the injection is given, or any new symptom. These are not all the possible side effects.
You may report side effects to the FDA at 1-800-FDA-1088 or www.fda.gov/medwatch.
Pregnancy and breastfeeding
Vitamin B12 needs increase during pregnancy, but adequate and well-controlled studies of cyanocobalamin injection in pregnant women have not been done, and vitamin B12 passes into breast milk. There is no FDA-reviewed information on the combined Lipo-B injection during pregnancy or breastfeeding. Tell your provider if you are pregnant, plan to become pregnant or are breastfeeding.
Lipo-B is not FDA-approved to treat vitamin B12 deficiency. According to the label for cyanocobalamin injection, giving vitamin B12 without a clear diagnosis can hide the true cause of anemia, and folic acid is not a substitute for vitamin B12. If a deficiency is suspected, your provider confirms it with blood tests.
