Subscription Terms
These Subscription Terms describe how treatment plans purchased through Forever Health Rx are billed, renewed and cancelled. They are part of the Terms of Service. The complete price of your plan is shown on the treatment page and again at checkout before you authorize payment.
1. What you are buying
A plan is a prepaid course of treatment for a fixed length (3 months, or month to month) that includes the medication and supplies prescribed for you, individual review of your file and ongoing access to your provider through the patient portal, and shipping. The online visit is free. Payment for the plan you choose is collected at checkout, before a licensed provider reviews your file. If treatment is not prescribed, nothing is dispensed and your payment is refunded in full, as described in the Refund Policy. Paying for a plan does not guarantee a prescription.
2. Prices
- Prices are in US dollars and are the same at every prescribed dose. There are no separate enrollment, consultation or membership fees. Any item not included in your plan (for example laboratory tests ordered by your provider) is disclosed and authorized separately before it is billed.
- The plan price is shown on the treatment page and again at checkout, where you confirm it before authorizing payment.
- Prices may be updated for future renewal periods with at least 30 days' notice by email; you may cancel before the change takes effect.
3. Automatic renewal
At the end of your plan length, your plan renews automatically for the same length and your payment method on file is charged the plan price on the renewal date, unless you cancel beforehand. Your next billing date is shown in your patient portal and in each billing notice. Renewal of a medication plan is conditioned on your provider's continued determination that treatment remains appropriate; your provider may pause or stop treatment at any time, in which case prepaid shipments not yet prepared are refunded.
4. Payment authorization
By providing a payment method you authorize to charge the amounts you have accepted at checkout and at each renewal until you cancel. If a payment fails we will notify you and may retry; medication shipments are not sent while a balance is unpaid.
5. How to cancel
You can cancel at any time from the patient portal or by emailing , with no contract and no cancellation fee. Cancellation stops future renewals and shipments; to avoid being charged for the next period, cancel before your next billing date shown in the portal. Refunds for the current period follow the Refund Policy. We will confirm your cancellation by email.
6. Insurance and HSA/FSA
Insurance is not required. Services are self-pay; we do not bill insurance and are out-of-network with all plans. Certain eligible medical expenses may qualify for HSA/FSA reimbursement; eligibility depends on your plan, and consultation, medication, shipping and laboratory components may be treated differently. Ask your plan administrator.
7. Records
Receipts, invoices, consents, prescriptions and care information are available in your patient portal.
